Lottery Signup Sheet - To participate, please fill out and return to the Foundation office.
 
How to play: 
Forward your application to the DH Foundation office or fax to 604-940-9670. Odds of winning and value of prize depends on number of entries. You can increase your chances of winning by purchasing additional entries.
 
Rules of Play:                                                       
  1. You must be a regular full-time, part-time or casual employee of Delta Hospital and at least 19 years of age.                                                             
  2. A report of payroll deductions is provided to DH Foundation by FHA Payroll each pay period. You can only receive an entry in pay periods in which you have earned income. Staff will not receive entries during unpaid leaves.                                                                  
  3. You are responsible for keeping your employment status and contact info up to date with the Foundation.
  4. Winners are required to pick up their cheque in person at the Foundation office and present photo ID.    
  5. Lottery participation is not considered a donation by the Canada Revenue Agency and is not eligible for a tax receipt.

When will the draws take place?
  • Draws are held on payday Friday's at 10am in the Foundation office.
 
Who is eligible to win the Staff 50/50 Lottery?   
  • Any full-time, part-time or casual employees working at Delta Hospital are eligible to enter the draw. DH Foundation staff are not eligible to participate.
 
Who sets the rules?
  • The Staff 50/50 Lottery is licensed by the Gaming Policy & Enforcement Branch and is operated in accordance with their rules.
 
When can I enter and for how long?
  • The Staff 50/50 Lottery is open for new applicants year round. Deductions from your pay cheque will start shortly after your completed form is received by DH Foundation and will end only when you choose to withdraw.
 
What does DH Foundation do with the money raised?
  • The DH Foundation will use the money it raises to purchase critically needed equipment as identified by the various departments in the hospital.
